The Health and Safety Executive (HSE) investigation revealed that Southampton Container Terminals failed to conduct a comprehensive risk assessment and did not implement a safe system of work at height. Despite known risks, they did not provide adequate safeguards, leaving employees exposed to serious hazards. HSE Inspector Francesca Arnold emphasised that employers must prioritise safety to prevent life-altering incidents and comply with safety regulations.
